Carr Gomm is a third-sector social care provider within the pathway model. The pathway model provides six different grades of support depending on the needs of the person supported, with Grade 6 being the highest grade, requiring hospitalisation, and Grade 1 the lowest, requiring minimal support in community settings. Carr Gomm is the only social care organisation in Scotland that supports people on each of the six graded pathways – from preparing people for discharge from the hospital to providing minimal support to people in their own homes. We are also the only service provider to be the lead partner on Grade 5 supported accommodation.
