Kotitori is a platform for the integration of social and medical care for older persons. With a location in the municipal hall, a phone centre, and an online portal, Kotitori functions as a "one-stop shop." This platform, which was developed as a joint venture between the public and private sectors, represents a new method of organising the planning, commissioning, and delivery of services for older people, particularly for home care.

The platform allows users and/or family members to select from a wide choice of providers. Kotitori makes information about the caliber of care available to assist in the decision-making process. Either the public or the private sector may offer services.

To support older people personally and at home, Kotitori coordinates a variety of measures, including:   Conducting user needs assessments and offering advice; Planning and monitoring care services (such as safety standards, IT systems); Offering feedback and conducting service audits; and coordinating peer-learning activities between service providers.

Aims of the project: A one-stop shop for older people and their relatives to provide information and guidance; Easing access to services; Improving the provision of information; Increasing the hours of work in direct contact with users in home care; and Enabling older people to live at home for as long as possible and to cope with their daily-life activities.
