We are engaging with our members to identify effective strategies for implementing integrated care and support in practice. We are doing this in the framework of a working group on ‘Integrated Care and Support’ which runs from 2018-2021.
The secong meeting, focusing on care leavers, took place in Edinburgh 26-27 September 2019 and was organised in collaboration with our Scotish members: Care Inspectorate, Social Work Scotland, Renfrewshire County Council and CELCIS.
The coordination of support between different sectors (social, education, health, police, employment, housing etc) to provide a more integrated response to care leavers is being increasingly recognised as important for:
- Addressing the complex issues faced by young people leaving state care
- Harnessing the expertise of professionals in different sectors
- Improving the efficiency of services
There are many different ways in which this can be achieved in practice. Some examples include:
- Joint working at strategic level on planning and decision-making
- Centre-based service delivery, where professionals from different agencies are co-located
- Multi-agency teams, involving professionals from different agencies working together on a day-to-day basis
- Case-management, where a professional ensures access to all services a young person may require
