If an individual’s unique needs cannot be met with existing community-based social services, this service is provided to meet those needs. The individual program regularly assesses the social situation and functioning abilities and needs of the person (family). This program may take place at a person’s (family’s) residence. The person’s (family’s) income is not assessed. Within this program, a person can be provided with housing, 24-hour monitoring, consultations according to their needs, support from specialists in solving special problems, communication support, and integration assistance.

The objective of this project is to ensure the person (family) has developed skills to organize an autonomous life within society (with or without support), be socially independent, and successfully integrate into all aspects of society.
