This practice improves resident monitoring and timely access to healthcare by utilising digital tools in nursing homes.
Care homes can enhance the quality of referrals to healthcare services and have better data sharing thanks to this straightforward IT solution. The goal is to increase resident results and minimise needless hospital admissions, which can be harmful to patients.

Prior to 2020, this work was done to increase access to health services, help with remote resident monitoring, and advance the digital maturity of nursing homes. 

However, in response to the Covid-19 outbreak, it was quickly scaled up throughout all senior living facilities in the Durham County Council local authority region to decrease unnecessary foot traffic and enhance infection prevention and control. This project aims to ensure residents receive medical treatment more quickly and stop their health requirements from worsening by increasing staff productivity, giving staff more time to care for people, and decreasing idling hospital admissions.
