The aim of this project is to support all children placed in and out of home care in Malta. To achieve this, an individual care plan is established to outline the needs of the child are met. Furthermore, this project ensures that the child has stability and independence growing up. Professionals are involved in detailing the individual plans as the goal is to see the child return to their family homes if this is in the best interest of the child. If reintegration back home is not possible, this project focuses on other solutions such as fostering or adoption. However, stability is the main goal.
Furthermore, this project attends to young people who are to leave the home care system. Continuous support and information are provided to the individual to ensure they can achieve an independent and stable life in society. Communication lines are kept open between the young person and the social services to ensure support is provided when accessing the services needed to improve their lives.
