The national initiative "Better Life for Older People" aims to improve health and social care by taking a holistic view of the older person's situation and needs. However, there is still a lack of cooperation among caregivers, and individuals are not involved sufficiently. Gaps in information and communication among the actors involved are also a problem.

Ume Municipality, in collaboration with the County Council of Västerbotten, launched the Multi7 project to address these issues. The approach of Multi7 aims to achieve the following outcomes: well-informed older people and relatives/caregivers; high quality health care interventions for older people; and effective coordination among health care providers.

They identified seven focus areas for improvement at the start of the project that could be pursued to achieve increased quality and efficiency. They have identified two basic working processes over time: safe and secure at home and safe and secure discharge from hospital. One district in Ume was used as a testing ground for Multi7's pilot project, where employees from the health centre and municipal home care services collaborated to create the Multi7 model. It began in November 2011 and ended in September 2013, with 80 people (with an average age of 84 years) participating, all of whom had a healthcare contact and were at risk of falling.

The project stakeholders all agree that Multi7 has resulted in a number of positive changes for both the elderly and their family members/relatives, as well as the staff involved in primary care, home care, and inpatient care. The project's findings indicate that the most frail older people frequently require extensive, holistic health care as well as clear information about health care interventions.
