Pathways into Adulthood seeks to bridge the gap between childcare and adult care.

• This gap is a shared concern of four partners, which are different organizations from the four areas listed below: Care for people with disabilities; childcare; general health care and welfare; and Local government.

• Funded from the budget of cross-sectoral activities centered on integral childcare.

• Five objectives are shared by the various partners: establishing and using a joint information folder; face-to-face recognition; drafting guidelines for professionals on “friendly transferral” of users between different services; development of outreach care run on the basis of a shared methodology; research on transition into adult care

A variety of actions were taken to carry out the project, including: professional networking opportunities; the creation of a joint information folder; the sharing of best practices among the four partners; and a workshop for professionals on transferrals.

Based on the evaluation, the following are the project's conclusions:

• increased knowledge and trust • ongoing sharing of best practices • shared information folder • early, compassionate, person-centered transferrals.
