Northamptonshire County Council took a different approach than other established service models: the local actors were motivated by the desire to improve care within a specific area by involving care professionals and fostering a new team culture. This will involve practitioners from multiple sectors, including law enforcement, education, and health care.

Northampton County Council develops and plans services in accordance with an innovation cycle based on the concept of co-production. The stages of the cycle are as follows: define; discover; dream; design; deliver; define.

This co-creation process was organized through open meetings and a 250-person summit. The meetings and summit were centered on sharing successful transition stories. Appreciative inquiries regarding needs and aspirations were used as a method to effectuate long-term and visible change.

Aims: Improving transition periods by acknowledging that transitions are critical stages in users' lives; Reshaping services and facilitating better cooperation between professionals as well as between professionals and users in order to stimulate change.
